Our role and the information we hold
Renova Dental is a health information custodian under PHIPA. Personal health information may include dental and medical history, treatment records, X-rays, images, insurance and billing details.
Collection and use
Information is collected directly from patients whenever possible and may be used to provide and coordinate care, communicate within the circle of care, process claims, manage the practice and meet legal or regulatory obligations.
Consent and your rights
Patients may request access to or correction of their records, ask how information is handled, and withhold or withdraw consent for certain uses subject to legal limits.
Safeguards and retention
Administrative, technical and physical safeguards protect information. Records are retained and securely destroyed according to RCDSO and legal requirements.
